2017 – 2018 Respite Club Membership Applications

Respite Club Applications become available in mid-August each year and are now closed for the full 2017-18 Respite Club season membership. If a family is only interested in Parent’s Night Out or “Open” Registration (November/March camps), please email michelle.elble@campcamp.org to request the application link.

Families must be members of the current season (September – May) to be able to register each month for Respite Weekend/Spring Break Camp, Parent’s Night Out (PNO) or Teen & Adult Day Adventure (TADA) events.

2017-18 CAMPer Respite Application Process

Applications will be processed for approval once you have completed the 3-Step Application Process:

Step 1: Submit an Online Respite Club Membership Application

Beginning in mid-August, you’ll be able to access the NEW application on the CAMP home page at www.campcamp.org under “CAMPer Applications” at the top of the page.  Under the “Applications” section click, “Respite Club Membership Applications” and the link will be at the top of the page. After beginning an application, be sure to check your new Parent Dashboard (under the Forms tab) or your email for additional guidance through the online and paperwork submissions.

Step 2: Submit Respite Club Membership Fee*

For your convenience, Respite Club Membership Fees may be paid online with the online application or via the My account tab. If you prefer to mail in your check or credit card payment, please print the 2017-2018 Respite Club Membership Payment Form and mail it to PO Box 27086, San Antonio, TX  78227.

*Membership fees are waived for campers who utilize funding from a TX Medicaid Waiver Program or are only attending Open Respite Camp events (in November OR March). Select “Other” and “Pay Nothing Now” at checkout if this applies to your camper, or if you’re unsure. Be sure to complete the print the 2017-2018 Respite Club Membership Payment Form with FULL Agency information and upload, fax, or mail it in.

Step 3: Submit Physician’s Report

After you submit your camper’s online Respite Club Application, you will be able to download the Physician’s Report form from the Forms tab.

  • Print the Physician’s Report form and have camper’s doctor fill it out based on an annual physical exam.
  • Upload the form into your camper’s application OR fax completed form to (210) 671-5244 OR mail it to PO Box 27086, San Antonio, TX  78227.
  • CAMP will not accept Physician’s Report forms dated prior to October 2016; a new physical exam must be completed if the date of exam is over 1 year old. If you believe a current physical is on file from a previous CAMP application, email Christina Gill at christina.gill@campcamp.org to check. If your camper’s physical will be expiring during the school-year, be sure to let us know the next scheduled exam date.

Respite Club Membership Applications will be processed within 5-7 business days from the receipt of the online Application, Physician’s Report and Paid Membership Fees. Upon acceptance into the Respite Club, CAMP will send an email with the 2017-18 Respite Club program policies and registration information for the monthly PNO, TADA, and Respite Weekend events.
